CHARACTERISTICS OF THE WORK
Under the direction of the FHC Director of Population Health and Quality Improvement, coordinates continuous quality improvement efforts and assists in care opportunity outreach to provide high quality care to Family Health Centers (FHC) patients.
Responsible for planning, implementing, and evaluating continuous quality improvement programs.
Ensures that electronic health records (EHR) information and FHC clinical data are accurate and are presented correctly in clinical measures reports.

Investigates internal reporting validity by completing chart audits and manual data review, ensuring accuracy of data.
Monitors implementation of any corrective measures recommended for improvement and coordinates chart audits to determine effectiveness of the corrective measures.
Monitors and enters patient data, such as preventative care screenings and health measure data, into the FHC computer systems to enable follow-up and reporting.
Communicates quality improvement information and clinical measures data in quality improvement meetings and to executive personnel.
Assists the FHC Director of Population Health and Quality Improvement with quality improvement activities.
Uses registry and other reliable information sources to inform care team members of the preventive care required for patients.
Conducts outreach to patients due for preventative care screenings and follow-up appointments.
Schedules patient appointments to eliminate care gaps.
Identifies patients with limited English proficiency (LEP) and/or other communication barriers and uses the appropriate interpretive services.
Scans documents into the patient’s electronic health record (EHR) as applicable.
Performs related work as required.
Minimum Requirements
MINIMUM REQUIREMENTS
High school graduation or its equivalent.
Five years of experience working in a healthcare setting, which must include at least one year of experience in a federally qualified health center (FQHC) and one year of experience in quality improvement.
An equivalent combination of training and experience may be substituted as determined applicable by Civil Service.
PHYSICAL REQUIREMENTS AND WORKING CONDITIONS
Lifts and moves materials weighing up to 10 lbs.
Spends extended periods of time sitting and reaching.
Spends extended periods of time typing and reading computer screens.
Identifies electronic and paper medical record documentation.
Responds to patient and Family Health Centers’ needs via the telephone, internet, fax, computer software and in person.
SPECIAL REQUIREMENTS
Must be available to work extended hours, including mornings, evenings, and Saturdays, as necessary.
Must provide own transportation to work at other FHC locations as needed.
