BASIC FUNCTION:
The purpose of this position is to oversee program monitoring and clinical outcomes data that support improvement initiatives for a HIV/AIDS healthcare clinic. The position is responsible for providing data entry support, analysis, and reporting. The position involves considerable responsibility, complexity in management. Coordination of all data-related activities, including for HIV-specific research programs. Must be familiar with a variety of the field’s concepts, practices, and procedures; Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks; A wide degree of creativity and latitude is expected and is accountable to the Ryan White Principal Investigators, Program Manager, and HIV/AIDS multi-disciplinary team, including Medical Case Management.
SUPERVISORY ACCOUNTABILITY:
This position is responsible for data collection, analysis, dissemination, and reporting of quality & outcomes data. Responsible for gathering data on performance metrics, facilitates the review of program effectiveness, and develops action plans to improve metrics and integrity of the data. The holder of this position will evaluate patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Outreach/re-engagement will be conducted within the District of Columbia communities that are considered high risk for HIV/AIDS.
NATURE AND SCOPE:
Internal contacts include administrators, faculty, staff in the department and University-wide. This position would serve as a liaison for quality clinical service delivery by providers with a particular focus on successful patient health outcomes and retention. External contacts include government agencies, sponsor representatives, vendors, visitors, officials, and the public.
PRINCIPAL ACCOUNTABILITIES:
Establish and implement data collection policies and procedures, including:
Develop a strategy and implementation plan, in conjunction with co-PIs, and as needed, HRSA National Quality Center, for streamlining data collection across HIV clinical providers.
Coordinate activities with a multi-disciplinary team. Determine demographic and clinical variables for monthly data collection.
Works with providers and clinical staff to identify cohorts of patients at-risk for health complications, poor adherence to HAART and experiencing gaps in care; and initiates quality improvement efforts to improve patient outcomes
Evaluates patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards; brings areas of unsatisfactory performance to the attention of the clinical staff and PIs.
Assess capabilities, value, and feasibility of Allscripts™ electronic health record system and CAREWare for program monitoring, data collection and clinical outcomes assessment.
Oversee all Continuous Quality Improvement Activities for Ryan White Programs, including management of CAREWare data systems.
Conduct monthly data analysis of program indicators and quality measures this includes implementing a data monitoring and reporting strategy according to HRSA and DOH standards and procedures.
Oversee clinical data analysis and as needed, collaborate with university statisticians for statistical support .
Perform specialized administrative and academic functions, including research and analyses, program coordination and monitoring, data collection, surveys, and reports.
Prepare ad hoc reports, records on program activities, progress, status or related special reports for the PIs.
Convene bimonthly quality meetings with and updates for a multi-disciplinary team, with subsequent decrease in frequency as guided by program and team needs.
Maintain a visual dashboard for the clinic.
Create and maintain reports based on dashboard metrics as requested by the division.
Work with selected providers to verify and improve the accuracy and integrity of the data.
Actively participates as a member of the Quality subcommittee by supporting decisions, accepting change, managing conflict effectively, and valuing the contribution of others.
Actively supports the strategic mission and vision of the organization by contributing to projects related to the coordination of care, patient retention, improving the patient experience, and ensuring the delivery of high-quality care.
Regularly update and maintain data records to ensure all information is current and accurate.
Verify data accuracy by cross-referencing patient records, forms, and reports, resolving any discrepancies promptly.
Perform regular data quality checks to identify errors or inconsistencies in patient records.
Assist with data audits and reviews as required, maintaining compliance with clinic and federal reporting standards.
Assist in preparing data for submission to funding agencies, adhering to reporting timelines and accuracy standards.
Greets and directs patients for appointments and general information, utilizing excellent and effective customer service skills.
Ensures adherence to Howard University/Hospital policies, procedures, and standard systems.
Provides back up support to the Patient Services Representative (PSR) to answer the telephone, screen calls as directed, takes messages and schedules appointments utilizing the electronic scheduling system.
Also, confirms appointments and provides patient instructions verbally or in writing in order to prepare for appointments.
Ensures that patients are handled consistently and schedule appointments in a manner that.
Assume other duties and responsibilities that are appropriate to the position and area, and or assigned by the supervisor.
MINIMUM REQUIREMENTS:
Bachelor’s degree in Public Health or related field. Eleven years of related work experience may be substituted in lieu of educational qualifications.
