Position Summary:

Under the supervision of the Operations Director, and in concert with Friends Aware mission, vision, values, policies and procedures and relevant federal, state and local regulations, the Quality Assurance Manager is responsible for incident reporting, agency Plans of Correction, and Committee Chairs. This is an exempt position with a minimal work expectation of at least forty-(40) hours per week, Monday through Friday, with the possibility of additional hours if needed.

MAJOR AREAS OF RESPONSIBILITY

Agency Committees:

Serves as Chairperson of the Quality Assurance Work Group. Responsible for scheduling the meetings, devising the agenda, recording the minutes, and the completion of the agency’s Quality Assurance Plan through data submitted by the Standing Committees and Committee input.
Presents Internally Investigated and Reportable Incidents to the Incident Report Review Committee, schedules meetings, ensures proper Standing Committee Review forms are completed by the Committee and transfers the analysis of each to the IRRC as minutes each quarter.

Staff Training:

Ensures that all staff receive Incident Report training within designated timeframe, conducts training and completes training certificates.
Assist with Mandt training and annual recertification within designated timeframe, conducts training and completes training certificates.
Conduct and assist with Trauma Informed Care training as well as Choking and Dysphagia training within designated timeframe, conducts training and completes training certificates.

Incident Reporting:

Responsible for daily monitoring, classification and tracking of all Incident Reports including their submission to DDA and OHCQ including DDA Reportable Incident Reports, Agency Investigation Reports (AIR), Appendix 5 report, and Outcomes Report. Also closes out (i.e. noted as completed) Incident Reports once all reporting requirements are met.
Maintain documentation of observations, notes, emails or any other correspondence regarding Reportable Incident Reports.
Send Outlook appointment reminders to RCs, RMs and Directors for AIR due dates to ensure timely responses.
Assist with e-investigations and on-site investigations/surveys being conducted by the Office of Health Care Quality (OHCQ) and the Developmental Disabilities Administration (DDA). Ensure the entity has received all requested documentation in a timely manner.
Completes and tracks all Plans of Correction for the agency.
Follow up of plans of correction to ensure compliance.

Other:

Serves as a resource liaison for the General Membership Committee.
Attend the Western Maryland Regional Office (WMRO) Quality Enhancement Provider meetings scheduled by the Developmental Disabilities Administration (DDA).
Participates in the Training Peer Network and the QA Peer Network.
Displays sound judgment in exercising responsibility for the safety and well being of consumers/maintains a safe working environment at all times.
Assist consumers to make responsible choices regarding their individual rights in all aspects of their program.
Assist, as needed, in emergency/crisis management situations. Review staff use of restraint procedures when these occur.
Maintain confidentiality in all consumer matters.
Assist in investigation of alleged abuse/neglect of individuals, as assigned by the Executive Director, Supervisor and/or Department Directors.
Gather and submit documentation required for DDA/Liberty Health Quality Reviews as needed.
Advocate for individual rights of consumers.
Works cooperatively with the Human Resources Director in the disciplinary process for employee counseling’s and/or termination regarding issues related to reportable incidents and quality assurance.
Performs other duties as assigned by Supervisor.

Qualifications:

Ability to exercise independent judgment, evaluate complex issues/problems as they relate to consumer behaviors, identify solutions and develop an appropriate course of action.

Excellent time management and organizational skills.
Ability to read and interpret regulations.
Ability to receive and maintain certification as a Mandt System trainer within agency designated timeframe.
Proficient in Windows based software (ie: Word, Excel, Publisher etc.) and an ability to learn and effectively use other software applications within agency designated timeframe.
Must possess typing skills sufficient to be productive.
Valid driver’s license with a minimum of 3 years of driving experience and an agency approved driving record.

Education and Work Experience:

Bachelor’s degree from an accredited university or college in Psychology or related field preferred. Preference also given to those with experience in the field of developmental disabilities.
