We are seeking a detail-oriented and experienced professional to join our team as a Quality Assurance Specialist – Note Auditor for the Tailored Care Management Division. The primary responsibility of this role is to review and audit care management documentation to ensure accuracy, compliance with organizational standards, and adherence to regulatory guidelines. The ideal candidate will have a strong understanding of healthcare documentation, regulatory requirements, and experience in quality assurance within the Tailored Care Management field.

Key Responsibilities:

Review and audit Tailored Care Management (TCM) notes to ensure they are complete, accurate, and compliant with regulatory and organizational standards.
Verify that care plans, interventions, and outcomes are properly documented in patient records.
Ensure that TCM notes adhere to confidentiality and privacy regulations (e.g., HIPAA).
Identify and report any discrepancies or areas for improvement in documentation, making recommendations for corrective actions as needed.
Maintain and update audit tracking systems, ensuring accurate documentation of audit findings.
Work closely with care management staff to provide feedback and support for improving documentation practices.
Stay updated on the latest healthcare regulations, industry best practices, and changes to compliance requirements affecting care management documentation.
Prepare detailed audit reports and present findings to leadership and relevant stakeholders.
Assist in the development and implementation of training materials to enhance the quality and compliance of care management notes.
Participate in continuous improvement initiatives aimed at optimizing the quality and efficiency of care management processes.

Qualifications:

Bachelor’s or Master’s degree in healthcare, social work, or a related field.
Minimum of 2-3 years of experience in auditing, quality assurance, or care management within a healthcare setting.
Strong understanding of healthcare documentation standards, care management processes, and regulatory requirements (e.g., CMS, HIPAA).
Excellent attention to detail and ability to identify discrepancies or gaps in documentation.
Strong analytical, problem-solving, and communication skills.
Ability to work independently as well as part of a team.
Proficient in Microsoft Office Suite and familiarity with electronic health record (EHR) systems.
